How to run it
Preparation
Whole team, including the part-timers and the person who joins next month if you can. Missing people do not follow rules they did not write.
Ask each person to bring one thing that worked in a previous team and one thing that drove them crazy. Two stickies, written before the session.
Prepare the five columns and pre-fill Rituals with what already exists (standup, sprint review) so the discussion is about changing, not listing.
Print or pin the charter template of a team people admire, as a reference for the level of concreteness expected.

Past teams Everyone shares their two stickies. No debate, just listening. Cluster the 'drove me crazy' ones: those become the rules.

Mission Write 'we exist to ... for ... so that ...' in pairs. Two rounds of merging. If it could describe another team in the company, it is not done.

Values into behaviors For each proposed value, ask 'what would we see someone do?'. Keep only values that produce an observable behavior. Three to four maximum.

Working rules Response time on messages, meeting-free blocks, how decisions get made when the PM is out, how to say 'I am stuck'. One rule per sticky, thumbs up or down from everyone.

Rituals and Definition of Done Confirm or kill each ritual with its purpose and owner. Then write the DoD as a checklist a new joiner could apply alone.

Sign and schedule Everyone puts their initials on the board. Book a 30-minute review in six weeks, on the spot.
Pitfalls
Values written as nouns ('transparency', 'ownership') change nothing. If you cannot describe the behavior, delete the value.
More than eight rules and nobody remembers any. Vote them down to the ones the team actually broke in the last quarter.
A charter without a review date becomes a poster. Six weeks, then quarterly.
Afterwards
The charter is pinned in the team channel and read to every new joiner in their first week. Its rituals get scheduled the same day; the Team Health Monitor six weeks later checks whether they hold.
